📘 Enrolment Forms (Enrol Type)

These forms collect detailed information, so it’s important to keep them clear, consistent, and easy to follow.

Welcome Page

This is the first thing families see. It should reflect your school’s branding and clearly explain what the form is for.

Include:

  • School name

  • Form name

  • A short welcome message

  • Form fee (if applicable)

  • School contact info (phone & email) – shown on every page

🧾 Before You Start Section

Let families know what they’ll need to complete the form.

Include:

  • A list of required documents

  • Any important policies

  • Optional: Add extra info in the “More Information” section (leave blank if not needed)

🧩 Form Structure

Keep the form in a logical order. Capture basic contact details early in case the user exits before finishing.

Suggested Sections:

Student Details

  • Name, DOB, Gender, Entry Year/Level, Citizenship, Religion, etc.

Parent/Guardian Details

  • Name, Contact Info, Relationship, Occupation, Education, Address

Student Profile

  • Interests, Learning Needs, Medical Info, Emergency Contacts

Documentation

  • Birth Certificate, Immunisation, Reports, Visa/Passport, etc.

This message appears after submission. Let families know:

  • Their form was received

  • What happens next (e.g., email confirmation, school will follow up)


🚫 Unavailable Form Message

If the form is offline, explain why (e.g., enrolments closed, system issue) and when it will be available again.

Keep these forms simple and focused on the event.

Key Fields:

  • Parent/Guardian: Name, Email/Phone (for matching)

  • Child: Name, Last Name, DOB (for matching) Entry Year/Level (optional if already captured at this stage)

  • Event Attendance Status

  • Additional Guests (if needed)

No Available Events Message:
Let users know if events are full or not yet open, and what to do next.


📩 Basic Forms (Lead Capture)

Use these for early-stage interest like:

  • Enquiry Forms

  • Info Pack Requests

  • Prospectus Requests

Keep it short and simple. Only ask for:

  • Parent/Guardian: Name, Email/Phone

  • Child: Name, Last Name, DOB, Entry Year/Level

  • Delivery preference (email or phone)
