Due to your form being online and dynamic, you're currently unable to print a blank version of the form. If you need a colleague to review the form, you can:
Give them access to Funnel so they can view the form editor preview
Provide the form's live URL for your colleague to perform a test submission
Complete a submission yourself with test data
If you have a family that is unable to access the form online, you can:
Assist them by completing the form for them over the phone, or by inviting them in and working through it together. This will involve logging into the form with the primary parent/guardian's email address, and then asking them for the access code
Create your own paper copy form as a fallback. This can then be entered manually as a lead into Funnel
